Special Educational Needs (SEN) Assessment Team

The 0 to 25 special educational needs or disability (SEND) Assessment Team is a local council service responsible for co-ordinating education, health and care (EHC) needs assessments and producing EHC plans for children and young people aged between 0 and 25 years who have severe, complex and long term special educational needs. They arrange the placement of children and young people with EHC plans in school, make arrangements for any special provision needed and oversee the annual review process.

Referral information

Anyone can request an Education, Health and Care (EHC) needs assessment. Most requests come from the school or setting. They must have permission from the parent or carer or the young person themselves. We require evidence that despite the setting having taken relevant and purposeful action to identify, assess and meet the child or young person’s special educational needs they have not made expected progress. A multi-agency SEN Panel considers all requests for EHC needs assessments and the authority makes a decision on whether to start an assessment based on the evidence available and the recommendation of the SEN Panel. The SEN Panel also considers whether to issue a statutory or non-statutory EHC plan at the end of an assessment, if one is agreed. Decisions are made taking into account the guidance available in the SEND code of practice 2015.
