Intensive Support Service - Dorset Healthcare N.H.S.

The aim of the Intensive Support Service team (IST) is to provide a highly specialised service to adults with a Learning Disability, supporting good mainstream practice as well as working directly with individuals with the most challenging needs. The service will predominantly provide support to the Community Learning Disability Team to manage an individual’s complex needs or individual crises. This includes when required to support young people in transition, aged over 14 years, when the young person meets the other referral criteria. The IST will provide intensive levels of support where needed to maintain individuals in a community placement, alongside appropriately commissioned or natural supports.

A Learning Disability is described as a significantly reduced ability to understand new or complex information, to learn new skills (impaired intelligence), with a reduced ability to cope independently (impaired social functioning); and which started before adulthood, with a lasting effect on development. Our service covers Bournemouth, Poole and Dorset. There is a Dorchester office but this is not accessible for clients or carers, but can be contacted on 01305 751360 for non-urgent calls as the office isn’t always staffed.
Referral information
Professional and self referral. The IST will respond as appropriate to the individual circumstances of each referral but will always seek to keep the person at the centre of all decisions and discussion.

Special Educational Needs and Disabilities Local Offer

Support for children and young people with SEN and disabilities
Our whole service provides an intensive level of support to adults with a Learning Disability, working directly with individuals with the most challenging needs. The team will also work with mental health teams when people with a learning disability need their support. The team carry out visits in the community whenever this is the most appropriate way to support people. Where there are complex needs and the community adult and children learning disability teams require support, the IST can begin working with people from the age of 14 onwards. They cannot take direct referrals or deliver direct care until individuals are 18. From 18 the team may become involved in a crisis as well and they would respond with a visit within 5 days or within 4 hours if an emergency.
