Community Children's Nurses – Dorset County Hospital

The Community Children’s Nursing service provides care for children in the community who have a specific nursing need. The aim of our service is to help prevent hospital admissions where possible, support earlier discharge if appropriate and support families with their child’s treatment at home. The community nurses work closely with other agencies where appropriate. Children are seen with both acute and chronic conditions. The core team is supported by specialist nurses who cover Wyvern School, diabetes and palliative care. Service users must be registered to a GP within Dorset but out of area referrals are considered on a case by case basis. For end of life care a 24/7 service tailored to the child and families need is provided. Access to out-of-hours medical care is via the usual way (GP, 111, 999, A&E, etc) unless the child has been given a designated temporary or long term Open Access arrangement to Kingfisher Ward.
