Additional Information
Our service covers the Purbeck area. Our office is the Purbeck Social Services Local Office, 19 Bonnets Lane. The service has a range of easy read leaflets available. The team includes staff who are experts in supporting people with Learning Disability and includes Nurses, Social Workers, Occupational Therapists, Speech and Language Therapists, Physiotherapists, Psychology and Psychiatry. We also work with other health and social care teams to help them support people with a Learning Disability. A persons needs would be assessed by the most relevant team member, or jointly if required and needs would be discussed within the Multi-disciplinary meetings. If a decision is made for ongoing support, the appropriate workers will work alongside the person in developing clear outcomes for their care.

Support for children and young people with SEN and disabilities
Our service is specific to those with a learning disability. We also work with young people and the children’s teams to help get them ready for adulthood. Getting to know people early and ensuring a comprehensive handover makes it easier for everyone moving forward through adulthood. The appropriate team will work alongside the service user, Children’s Services and Ansbury, to develop clear outcomes for their care. During the 17th year of a young person, the transitions team will work with the adult team if further support is needed post 18 years, ensuring a clear package of care remains in place. Getting to know people early, and ensuing a comprehensive handover makes it easier for everyone moving forward through adulthood. You can contact Rachael Howes on 01305 221016 for more help and guidance or email adultaccess@dorsetcc.gov.uk.
