The aim of the Bournemouth Community Team For People With A Learning Disability (CTPLD) is to promote the health and wellbeing, independence and a better quality of life for adults with a learning disability. Service users will be able to access all mainstream services in accordance with their needs. Our service has the capacity to support service users in with a learning disability in the community, support family carers and provides specialist assessment and therapy.

Additional Information

Our service covers the Bournemouth area. The service has a range of easy read leaflets available. The team includes staff who are experts in supporting people with Learning Disability, including Nurses, Social Workers, Occupational Therapists, Speech and Language Therapists, Physiotherapists, Psychology and Psychiatry. We also work with other health and social care teams to help them support people with a Learning Disability. Our rooms are wheelchair friendly and we also visit people in the community. We work with young people and the children’s teams to help them get ready for adulthood following their 16th birthday. An assessment is then offered by the Transitions Social Worker following their 17th birthday. The Social Worker will work alongside the Children’s teams and all other relevant workers from health services and education. The assessment competed will be holistic with consideration to health, education and social care needs and will take consideration of the views of all those concerned with the young person’s care. Should the assessment identify ongoing needs all options will be considered to determine the most appropriate viable support before making a proposal and reaching a decision.

Referral information
Professional Referral and Self-Referral. The service provided by the team is for people with a learning disability. This has been defined in Valuing People as including the presence of: • A significantly reduced ability to understand new or complex information, to learn new skills (impaired intelligence), with; • A reduced ability to cope independently (impaired social functioning); and; • Which started before adulthood, with a lasting effect on development.
