Community Children's Nurses – Dorset County Hospital

The Community Children’s Nursing service provides care for children in the community who have a specific nursing need. The service covers children from 0 to 18 years of age who have a paediatrician at Dorset County Hospital. The aim of our service is to help prevent hospital admissions where possible, support earlier discharge if appropriate and support families with their child’s treatment at home. The community nurses work closely with other agencies where appropriate. Children are seen with both acute and chronic conditions. The core team is supported by specialist nurses who cover Wyvern school, diabetes and palliative care.

Service Details

Additional Information
Service users must be registered to a GP within Dorset but out of area referrals are considered on a case by case basis. For end of life care a 24/7 service tailored to the child and families need is provided. Access to out-of-hours medical care is via the usual way (GP, 111, 999, A&E, etc) unless the child has been given a designated temporary or long term Open Access arrangement to Kingfisher Ward. Messages can be left on the answering machine outside of hours on 01305 254279.
Referral information
Referrals are accepted from Consultant Paediatricians. In most situations the General Practitioner (GP) sends the referral.

Support for children and young people with SEN and disabilities
Children are seen with both acute and chronic conditions. This includes, for example, Cystic Fibrosis, complex care, palliative care, diabetes, oncology, respiratory conditions, wound care and nasogastric or gastrostomy feeding. The core team is supported by specialist nurses who cover Wyvern school, diabetes and palliative care.
