The aim of the Purbeck Community Team For People With A Learning Disability (CTPLD) is to promote the health and wellbeing, independence and a better quality of life for adults with a learning disability. Service users will be able to access all mainstream services in accordance with their needs. Our service has the capacity to support service users in accessing appropriate services and opportunities, support service users in the community, provide service user case management, support family carers and provide specialist assessment and therapy.

Our service covers the Purbeck area. Our office is the Purbeck Social Services Local Office, 19 Bonnets Lane. The service has a range of easy read leaflets available. The team includes staff who are experts in supporting people with Learning Disability and includes Nurses, Social Workers, Occupational Therapists, Speech and Language Therapists, Physiotherapists, Psychology and Psychiatry. We also work with other health and social care teams to help them support people with a Learning Disability. A persons needs would be assessed by the most relevant team member, or jointly if required and needs would be discussed within the Multi-disciplinary meetings. If a decision is made for ongoing support, the appropriate workers will work alongside the person in developing clear outcomes for their care.
Referral information
Professional Referral and Self-Referral. The service provided by the team is for people with a learning disability. This has been defined in Valuing People as including the presence of: • A significantly reduced ability to understand new or complex information, to learn new skills (impaired intelligence), with; • A reduced ability to cope independently (impaired social functioning); and; • Which started before adulthood, with a lasting effect on development. All referrals should be through Adult Access by emailing adultaccess@dorsetcc.gov.uk or telephone 01305 221016.

Special Educational Needs and Disabilities Local Offer

Support for children and young people with SEN and disabilities
Our service is specific to those with a learning disability. We also work with young people and the children’s teams to help get them ready for adulthood. Getting to know people early and ensuring a comprehensive handover makes it easier for everyone moving forward through adulthood. The appropriate team will work alongside the service user, Children’s Services and Ansbury, to develop clear outcomes for their care. During the 17th year of a young person, the transitions team will work with the adult team if further support is needed post 18 years, ensuring a clear package of care remains in place. Getting to know people early, and ensuing a comprehensive handover makes it easier for everyone moving forward through adulthood.
